Steven Nissen discusses ALPACAR (NCT05537571), the phase 2, five-arm, placebo-controlled trial of zerlasiran (Silence Therapeutics' SLN360), a double-stranded siRNA against LPA. 180 adults with elevated Lp(a) and high ASCVD risk were followed to 60 weeks; the primary outcome was the time-averaged change in Lp(a). Zerlasiran 300 mg every 16 or 24 weeks and 450 mg every 24 weeks produced time-averaged reductions above 80 percent, with mainly mild injection-site reactions.
